Our Privacy Policy explains how Affinity and its related entities (Affinity Healthcare Solutions LLC, Affinity Healthcare Solutions Skilled care, “Us”, “We”) collect, use and share Your information, including through our digital properties on which this Privacy Policy is linked (“Site”). All telephone calls with Affinity staff may be recorded and/or monitored for quality assurance purposes. By contacting or communicating with us in this method you agree to the recording for such purposes.

This includes emails, text messages, phone communications, voicemails, toll-free calls, chats, etc. even if you previously registered for a do not contact list. All SMS messaged are for Affinity’s internal uses only. Affinity does not sell your information.
Consenting to our communication policy (whether via telephone or SMS) is not an agreement to purchase or commitment to any form of contract.

Standard Messaging Disclosure

Many standard SMS carriers charge fees depending on your carrier’s contract and whether the communication is domestic or international. Affinity does not sell or share your information. Your consent to get SMS only applies to Affinity and its providers, this consent does not extend to any other third parties.

Your Rights

You have the right to ask for the information that we have collected about you whether from you or our interactions which we would abide by. This does not include any information which we may be required to maintain due to legal, administrative, and security purposes. Please call our office with any requests.
You have the right to request another method to be contacted or not be contacted at all. Any request to be contacted in another method must be provided in written form (whether my mail, email or texted). Affinity will try to accommodate any reasonable requests

As a home health agency, we are required to be compliant with various privacy frameworks including HIPPAA, Department of Health, CMS, Accreditation bodies, Insurance companies, etc.  The information we received may be used for:

  • Affinity’s Business Operations (Care coordination, Billing, Client Insight, Marketing, Analytics, etc.)
  • Emergency care coordination
  • Usages required legally or by a court
  • National Priorities Coordination and Reporting (neglect & abuse required reporter mandate)
